incident template: match existing template

JHathaway requested to merge jhathaway/corto:update-template into main

To reduce the congnitive load of moving to corto, match the existing template as much as possible:

  1. Download existing template as markdown
  2. Replace tables with bulletted list, to ease further changes
  3. Tidy up a few other pieces, add italics, etc
